Provision of Services

Prior to collecting signatures on the Individualized Plan for Employment (IPE), the VR counselor and project director will discuss the rationale for the IPE, as well as the director’s recommendations for and/or changes to the IPE. During this consultation, the project director and VR counselor must consider the relevance of comparable services and benefits for provision of services. Once the IPE has been signed by the VR counselor and the project participant (and/or designated representative), the VR counselor must complete the following steps:

  • Retain the original IPE in the participant’s file and provide a copy to the participant or representative, as well as a copy of the participant rights and responsibilities form.
  • Conduct a formal annual review of the IPE with the project participant. Note: The IPE may be reviewed more often if necessary, at the request of the participant or the VR counselor.
  • At the time of the annual review, assess the participant’s progress toward achieving an employment goal, and amend the IPE if necessary.
  • Document the annual review in the case record.

An IPE amendment may occur any time after the IPE has been finalized and approved. It might be appropriate to amend the IPE if changes are made to any of the following:

  • the employment goal
  • the VR services to be provided, including comparable services and benefits
  • vendor(s) for an identified service
  • extension in project completion dates
  • change and/or expenditure for services
  • change in participant’s case status
  • change in participant’s disability condition

In amending an IPE, the VR counselor will apply the project participant’s informed choice provisions and obtain approval from the AIVRS director for all amendments. Any additional services, new services, or the extension of existing services will be determined based on the changing needs of the participant, with a review of available comparable services and benefits by the participant’s VR counselor. IPE amendments do not take effect until the participant (and/or authorized representative) as well as the VR counselor and/or project director agree to and sign the revised document. Once signed, the VR counselor will (a) provide a copy of the amended IPE to the participant or authorized representative, and (b) retain the original copy of the approved IPE amendment to place in the case record.

  • Provide services based on the IPE in order to address impediments to employment, resulting in gainful employment for project participant.
  • Provide services that are equally accessible across the state offered by AIVRS and/or State VR agencies.
  • Examine the participant’s need for changes to services as required to achieve an employment outcome.
  • Help the participant obtain diagnosis and/or treatment of impairment(s).
  • Complete and/or help the participant complete any required assessments.
  • Provide job search assistance, job placement services, and on-the-job supports as needed.
  • Provide counseling and guidance throughout the AIVRS case.
  • Provide any employment goods or services (clothing, equipment, supplies, etc.).
  • Direct participant to complete any required vocational training, or other services as needed.

  • Authorization of Services (AIVRS project policy and procedure)
  • Obtain AIVRS project director or supervisor approval of services over $500 (or any other amount indicated by AIVRS project policy and procedure).
  • Amend IPE if necessary.
  • Review non-allowable costs with participant.
  • Close case if participant did not or could not seek employment.
  1. Note start date and completion date of VR services in the IPE.
  2. Verify economic need of project participant prior to services (if required by project policy).
  3. Maintain contact with project participant to assess participant’s progress towards employment outcome as noted in the IPE.
  4. Clearly explain to project participants that they must actively participate in their approved services as described in the IPE, in addition to making consistent progress towards achieving an employment outcome, or services will be discontinued.
  5. Identify comparable services and benefits such as Medicaid/Medicare; PELL grants or other available grants; additional non-merit-based scholarships; private or other available medical insurance; Veteran’s Administration services (for health care and rehabilitation center needs); worker’s compensation (when the person has been injured on the job); or State-funded childcare.
  6. Complete provision and outcome of the participant’s services case notes.
  7. Record how participant informed choice was exercised prior to provision of services case note.
  8. Record expected outcome of goods and/or services in a case note.
  9. Amend IPE if services are changed, deleted, completed, or discontinued.
  10. Authorize payment for goods/services per project policy and procedure.
